Waiver
HOUSTON AUDUBON SOCIETY
WAIVER AND ASSUMPTION OF RISK AGREEMENT
I acknowledge and fully understand that there are certain dangers, safety hazards and risks inherent in outdoor field activities. These include but are not limited to the risk of loss of life, personal injury and loss or damage to personal equipment. I certify that I am physically able to participate in this activity.
In consideration of Houston Audubon (HA) providing the opportunity for me to participate in this activity, I assume full responsibility for all risks of injury or loss of life to myself and loss or damage to my own equipment when participating in this HA activity. I hereby release, forever discharge and waive for myself, my heirs or anyone claiming through me, my right to sue the HA, its officers, staff, trip organizers, leaders, instructors or any HA members or guests, for any cause of action, claims or demands for injuries to my person or equipment which may occur during any HA activity.
This waiver applies to any negligent act or omission and to any intentional act intended to promote my safety or well-being.
I acknowledge that I have read the above carefully and understand the legal consequences of this WAIVER and ASSUMPTION OF RISK AGREEMENT. I have been informed of the prerequisite skill level required for this activity and certify that I possess the skill and competence to safely participate in it.
